NOTE: If you have Medicare (or whether it is primary/secondary to another commercial insurance), you can still be seen as a private pay client but you will need to sign a simple private contract (required by law for out of network providers) acknowledging that neither you or I will seek reimbursement from Medicare. This allows for longer appointments and more direct access.
How To Submit a Medical Receipt or Superbill
To Your Insurance (applies to those with commercial insurances only, not combined with Medicare)
We do not participate in commercial insurance but we can easily generate a superbill/receipt of services for you to submit to your commercial insurance company for potential reimbursement. This applies if you have Out of Network (OON) benefits with your insurance, so check with them first.
To get reimbursed, you first pay your provider upfront. At the end of your appointment (if you request) we give you a superbill and you submit the superbill along with a member claims form on your insurance’s website. They might also offer submission by fax or regular mail.
While our practice is not responsible for out of network reimbursements, here are steps to consider:
call the member number found on your insurance card.
tell your insurance provider that you would like to submit a superbill for psychiatric services.
ask how much will they cover for an out of network psychiatric evaluation and ongoing psychiatric services.
ask where you can send the superbill (ie, fax number), and how you can follow up to ensure they received it.
the insurance company should then send you the reimbursement money. Ask what the timeline is for receiving your reimbursement, confirm they have your address (or how you will get it), and what number you can call or how you can track your payment.

Blue Cross Blue Shield of Texas: submit your superbill online by first becoming a Blue Access Member. Once you are established as a member you can go to the online portal and navigate to the claims section to submit your superbill. https://www.bcbstx.com/docs/forms/claim/tx/medical-claim-tx.pdf
Or you can mail or the completed form and your itemized superbill to Blue Cross and Blue Shield of Texas, P.O. Box 660044, Dallas, Texas 75266-0044. Ensure it is submitted within the allowed timeframe (usually 18 months for HealthSelect plans, or 95 days for other standard plans) Fax # 1-877-886-2593.
