2. If my treatment approach is compatible with your goals I will send you a scheduling link to your email address

3. Once you receive the link, click on it to make an appointment with me (click on “New Intake Initial Appointment” if we have never met, “Follow Up Appointment” if you are already known to me)

4. Bring your last 3 progress notes from your previous mental health provider and your last set of bloodwork results to your appointment with me. If I was your psychiatrist at Baylor Scott & White — call 844.848.BSWH (2794) to obtain your records)

5. Your first appointment must be in person because of paperwork to review and sign.‍ ‍
